Drawings for Early Bird Cash Prizes will be held at Centerville H. S. home football games on Sept. 10, Sept. 24, Oct. 15 and Oct. 22, 2004.

STEPS TO ENTER RAFFLE:

1. Read Rules & Regulations

2. Click on Entry Form

3. Print out form on your printer.

4. Fill out form: # of tickets, Name, Address, Phone, etc. To pay by credit card, fill out Credit Card Purchase box - MC & Visa ONLY.

5. Mail entire form to:

             Centerville, OH 45441-0134
   If paying by check, enclose signed check -SEND NO CASH

6. To confirm receipt of your entry, enclose self-addressed stamped envelope.  OR your cancelled check or credit card charge will serve as your receipt. No actual ticket will be sent.

7. Monitor this website for winners!
